• What will I need for my first appointment?

Comfortable clothes or change of clothes; prescription or referral (if you have one); any recent images (X-rays, MRI, etc...); specific equipment related to your problem, for example, runner's should bring their running shoes, insurance card, if you want us to submit claims to your insurance company.

If you have completed your forms please arrive 15-minutes early, if you have not completed your forms please arrive 30-minutes early.
